From Interstate 90 & Route 11

From Route 90, take Route 11 North to the East 21st Street exit, take E 21st Street west to Columbus Ave., follow

Columbus Ave south to the light, turn right on East 23nd Street, at the next stop sign turn left onto Harbor Ave., immediately turn right at East 24th St. We are on the north side (right) of the road. Blue and white hospital signs mark the route beginning at the East 21st Street exit. Our facilities are located across from the Ashtabula hospital.

From Route 20
At the intersection of five points, turn on to Lake Ave., follow Lake Ave. to the intersection of East 24th Street, just past ACMC Medical Center. Turn right on to East 24th, we are on the north side (left) of the road.
